Note - form prints odd - doesn't fit on one page. Right click on 'image' - select Print Picture and then print. This worked for me.

Did you notice it says over 100+ eligible products, but there are less than 20 items to choose from on the rebate form?

Did you also notice that it states "receipt" and not "receiptS" as in more than one.

Near the bottom of the form (after you put in your contact information) it says to mail completed submission form, UPC codes and cash register receipt(s)...

However, I do see the other receipt you mentioned.

I am registered with BTFE, but I was not offered this rebate ??!! Guess I am out of luck.

Could you please post (or PM me) the address the form is to be sent to? I have had a difficult time printing the form (I received the email link.) I have had no success calling Customer Service.

I ended up printing a garbled rebate form. I'm going to send it in with a note explaining the problems I had printing it (fingers crossed).

I'll check the form tonight and let you know. I had trouble printing it also. I ended up doing a right mouse and then saying print picture for it to print properly.

Does anyone think it can be a copied receipt? Doesn't say "original" receipt.

Does anyone know if this has to be all on one receipt or can be on separate receipts. i don't see any number to call and the top of the form says receipt, the bottom says receipt(s).

Answering myself. Just called them and asked. was told that this can be done in multiple transactions.

I messed up and bought only 9 items between 2 receipts. Would anyone happen to have an extra receipt with just 1 of the qualifying items plus the UPC code for it? Would hate to lose out on this for me and the school. Thanks!
