I called the customer service # and they said you don't need an official form, but you need to make the purchase from a participating store.

Last year CVS, Walgreens, Rite Aid, Walmart & Target were NOT participating, and to be on the safe side again, I would recommend not purchasing from them.

Send: Name, address, zip code and phone number, along with the one CRT with your $25 in P&G purchases circled, dated between 9/28/11 through 10/31/11 and postmarked no later than 11/7/11.

at least we can still use coupons to get the total out of pocket expense lowered

so i can buy just one box of crest whitening strips for $25 and I would still qualify?

Sure I don't see why not. But the thing is you have to purchase from a "participating retailer" which is why I posted the reabte. I would like to know what other retailers people know of that are participating

G&G Market (which is a small Grocery store chain north of the San Francisco bay area) is one of the participating stores. Even though it may help only a handful of people, I figured I would contribute.

Does it have specific dates or items you have to buy?

When I called last year I was told it was mostly small, regional chains that were participating. From what I've read online this year it seems like the same holds true for this year.

Toys R Us participated 2 years ago.
