Need a Rebate Form for Bausch Contact Lenses
Does anyone have the code to obtain the rebate form for the Bausch and Lomb Purevision HD2 contact lenses. I called and they said I need to get it from my doctor, but my doctor does not have the code. I hope someone on the forum is a user of these lenses and can help me out. The rebate form I have asks for personal code on the rebate form. If they offered the promotion they should have the code :/
Which eye doctor is it? Last edited by Rabb02; 10-27-2011 at 01:11 AM.. 2012: Lots of Campaigns so far

It was a Target doctor, who eventually gave us a code. Thanks anyway.

I'm glad you got the code. It looks like you have to have it to even submit the rebate anyways
