begins 9:00 a.m. PT on May 4, 2012 and ends 5:00 p.m. PT May 21, 2012.
Entrants may only receive up to five (5) total entries in this Sweepstakes regardless of the method of entry (or combination thereof) they choose.

PRIZES: One (1) prize (“Prize”) will be awarded in this Sweepstakes.
