Transitions Optical “Sight of the Day” Promotion ~ 9/30/12 (photo)
Promotion begins at 12:00 AM (Eastern Time [“ET”]) on March 8, 2012 and ends at 11:59 PM ET on September 30, 2012 (the “Promotion Period”), subject to entry period dates listed below.
In order to be eligible for entry into the Promotion, you must be a legal, permanent United States resident residing in the 50 United States or the District of Columbia and 18 years of age or older with a unique valid Social Security number at the time of entry.

Entrants can only add one photo at a time for submission. Multiple photos uploaded at one time will constitute multiple entries. If an Entrant enters multiple times, that Entrant can only submit an original photo one time. An Entrant will be disqualified if submitting the same photo more than once during the Promotion.
Entrants may enter as often as they wish for each Entry Period and the Promotion Period.
Each Entry Period will run from 12:00 am ET to 11:59:59 pm ET, according to the dates listed in Chart A, and all entries must be submitted during this time in order to be included for that Entry Period and subsequent Entry Period drawings.
All non-winning eligible entries from the previous Entry Periods will carry over to the next Entry Period and will be eligible for subsequent Entry Period drawings and the Promotion drawing.
ENTRY PERIOD PRIZES:
There are a total of twenty-two (22) Entry Period Prizes to be awarded during the Promotion. For each Entry Period, one (1) Entry Period prize winner will receive a gift tote package to include the following: i) Shutterfly® tote bag, ii) Scenic America Book, ii) National Geographic “10 Best of Everything” Book, iii) Southern Living Cookbook, iv) $50 Shutterfly® gift card, v) digital camera with case and memory card, vi) $250 gift card to be used towards the purchase of Transitions lenses, and vii) sunscreen. Approximate Retail Value (“ARV”) of each Entry Period prize is $560.
Total ARV of all Entry Period prizes is $12,320.
PROMOTION PRIZES:
GRAND PRIZE:
The Grand Prize winner must be a legal permanent resident of the 48 contiguous United States or DC and 18 or older to be awarded the Grand Prize. One (1) Grand Prize of a trip to the Galapagos Islands will be awarded (the “Galapagos Islands Trip”). The Galapagos Islands Trip includes roundtrip coach airfare and 9 nights/ 10 day hotel accommodations (one (1) standard room). Sponsor will determine the number of travel companions, hotel accommodations and dates of travel once a prize winner has been selected; however, the amount that will be awarded towards the retail purchase price of the Galapagos Islands Trip will be no more than $10,000. The Grand Prize winner will also receive a DSLR camera having an Approximate Retail Value (“ARV”) of $600. Total ARV of the Grand Prize is $10,600.
FIRST PRIZE:
The First Prize winner must be 21 years of age or older in order to be awarded the First Prize. One (1) First Prize of a trip for four (4) with Funjet Vacations to Maui, Hawaii will be awarded (the “Hawaii Trip”). The Hawaii Trip includes roundtrip value economy airfare for four (4) on a major carrier of Funjet Vacations’ choice from a major domestic U.S. gateway city closest to winner’s home and a five (5) night stay at the European Plan Hyatt Regency Maui Resort & Spa, Maui, Hawaii and $600 to be used towards passenger travel taxes. Sponsor will determine the number of travel companions, hotel accommodations and dates of travel once a prize winner has been selected; however, the amount that will be awarded towards the retail purchase price of the Hawaii Trip will be no more than $5,400. Total Approximate Retail Value (“ARV”) of the Hawaii Trip is $5,400. Winner is responsible to get to a Funjet Vacations’ departure city.
SECOND PRIZE:
One (1) Second Prize of a $1,500 American Express gift check will be awarded. Total ARV of the Second Prize is $1,500.
Total ARV of all prizes to be awarded is $29,220. Limit of one (1) Entry Period prize per person/household. Entry Period prize winners are eligible for the Promotion Prizes. Limit of one (1) Promotion prize winner per person/household.
For the Grand and First Prizes, the prize winner must complete travel by August 31, 2013. Grand and First Prizes are subject to availability and blackout dates. Travel must be roundtrip. Sponsor is not responsible for airline cancellations or delays. Confirmation will be given on a space available basis only, Once a reservation is made, no changes are permitted. All expenses and incidental travel costs not expressly stated in the Grand and First prize descriptions above, including but not limited to, passenger tariffs or duties, surcharges, airport fees, service charges or facility charges, personal charges at lodging, security fees, telephone calls, and tours and excursions and purchases made at the Specialty Shops and any other expenses or incidentals are the responsibility solely of prize winner. Any taxes imposed on the income of the prize winner as a result of winning a Grand or First prize and all other expenses not specifically mentioned herein as part of the Grand and First prize are solely the responsibility of the prize winner. All travelers must possess required travel documents (e.g. valid government-issued photo I.D. and/or passports) prior to departure. Travel companions must travel on same itinerary and at the same time as the prize winner. Travel companion must execute liability/publicity releases (except where prohibited) prior to issuance of travel documents. If travel companion is a minor (based on age of majority in jurisdiction of residence), he/she must be accompanied by his/her parent or legal guardian (i.e., the winner), and his/her parent/legal guardian must sign releases on his/her behalf. Travel restrictions, conditions and limitations may apply. Married or divorced women traveling under names other than what is printed on their travel documents must supply a marriage license and/or divorce decree. No transfer or substitution of the Grand Prize or First Prize is allowed except at the sole discretion of the Sponsor due to prize unavailability for any reason and only then for a prize of comparable value. There is no cash alternative offered, except at the sole discretion of the Sponsor. The prize winner is required to comply with any and all applicable federal, state and local laws, rules and regulations. The value of the Grand and First Prize is taxable as income and prize winner is solely responsible for all matters relating to the Grand or First Prize, including, but not limited to all applicable federal, state and local taxes that become due with respect to the Grand or First Prize. Any difference between the stated Approximate Retail Value (“ARV”) and the actual value will not be awarded.
