Classic plan also have this promo benefit, but monthly will be slightly more, BUT YOU GET BETTER DISCOUNT on smartphone. Value plan customer needs to buy smartphone at "retail price". In my location, costco retail price on smartphone is cheaper than t-mobile retail store.

HOW TO GET THIS DEAL
STEP 0: Check t-mobile coverage in your area: http://www.t-mobile.com/coverage/pcc.aspx
If you are new to t-mobile or getting a new plan, please do the activation in person, do NOT use t-mobile chat. This will save a lot of grief and human error, a wisdom from many experienced t-mobile customers.
A.If you are still in contract t-mobile customer, call t-mobile directly for the add-on promo. If you are interested in value plan, they will have to check if you qualify for value plan. Check migration fees below. For NEW customer, I would recommend going personally to costco or tmobile retail store. Update, just learn this myself on 7/5: You can get this online. Put the package into the cart first and then customize each line at checkout, http://www.t-mobile.com/shop/Pack...kages.as px. Update 7/6: if you cannot get unlimited plan via t-mobile website, you can call t-mobile directly or goto retail store (t-mobile, costco, best buy...etc). It's still available, t-mobile website is just missing lots of features and plan.
B. IF you are interested in value plan and planning to get a smartphone at costco, eg Samsung S2, do steps below:
1. First make sure you qualify: out of t-mobile contract or qualify for upgrade (not value plan) or less than 6 months left on t-mobile contract (not value plan) or new t-mobile customer or currently value plan customer wanting add-a-line, AND (access to costco membership and data plan)
2. Goto local costco. Tell them that you wanted value plan and wanted smartphones as well. Do your transactions. New activation is free, not sure about existing add-a-line customer. Sim is free.
3. Go home, make sure everything works and happy. If you have corp/military/eduction discount..etct, then call t-mobile CS. They should be able to apply for you.
UPDATE 7/23/2012: Costco will NO longer sell S3 with value plan.
UPDATE 8/07/2012: Costco now require phone purchase with all value lines activated. Thus I would recommend ONLY activating value plan lines for smartphones (first two line) or (add-a-line). Costco smartphone are decently priced but all their dumb phone are really really expensive! Then goto t-mobile stores to add/activate the rest. Remember to ask for free activation.
C. You DO NOT need to goto costco to get the value plan and classic plan deal. You can get value plan or classic plan along with smartphone anywhere, eg: t-mobile store, amazon wireless, walmart, ...etc. Major differences is the pricing of the smartphone. You can always buy at independent market like google nexus at google play or ebay or craigslist or amazon store. Costco is only mentioned here because they have great smartphone price when u activate/signup there. Where and when you activate/signup the value plan will determine the cost of your smartphone. So if you signup/activate at costco, then you get costo smartphone pricing. If you signup/activate at t-mobile store, you get t-mobile store smartphone pricing, you cannot go back to costco to get costco smartphone pricing later for that line. Lesson here: signup/activate plan or line where the smartphone is cheapest, and get the smartphone for that line at the same time.
D. If you don't want to buy smartphones for value plan, then just goto t-mobile store. It'll be free sim and NO activation fees. Note that once you are already in value plan, in future, you will NOT be able to buy smartphone for those lines at value plan prices, not in costco and not with t-mobile. Those lines will not be able to transfer back to a subsidized t-mobile plan (eg: classic, ...etc). This is the current t-mobile policy. It may or may not change in future. You can buy phones at ebay, craigslist, google play or get pre-paid phones, ..etc. Note that phone prices trend is dropping like crazy.
E. If you decide to buy smartphone, remember that tax on smartphone is based on retail price. If you pay migration fees, there will be tax on that too.
My experience shows that being nice to the salesperson or cs makes getting the deal easier, especially retention
