One of the guys at work tried this and said he busted some connector so the touchscreen stopped working after he replaced it. Thought I'd pass out this warning.
You obviously have to be careful, many of the connectors you remove have very tiny contact point which obviously could be damaged.
OP, I got mine from "gadgetfix" and I was happy with the results. Obviously it was not OEM, but the display looked identical to me and they had good feedback and were US based. It worked perfectly for over a month until my GF dropped the phone in the toilet. Then it worked for about two weeks and died permanently. I have not tried some of these full board scrubs that some people are selling as a fix, but maybe one day...
Someone I know tried replacing their screen and ripped some connector in there, causing the phone to be a brick. In another instance, a friend and I took a cracked iPhone to some ghetto cell phone store. They replaced the screen for $75. It took about an hour and the screen was perfect quality. You might want to consider that option.
So, $600 for an iphone 5, or $30 for replacement glass....

I don't mind digging around inside of the phone myself - I've done it before on the 3GS. Heck, if I can replace a hard drive in a unibody Macbook, I should be able to swap the glass on a 4s.
It's a little more difficult than that, doing a keyboard swap on a MBA was easier IMO. Just get yourself the spudger/spooger, whatever it's called, and it should help quite a bit. I'm an experienced tinkerer, by no means an expert electronics guy though, and the whole thing took me about an hour from start to finish.
