How long did it take for you to receive the code?
You should upload the requirements of the promo as shown in OP or call Time Warner at 1-888-TWCABLE.

I do not think that calling TWC will help since they do not know the deal and it does not show up on their promotion list. You might want to call the BB TWC rep instead.

Service installed on Nov 6th got gift card code today via USPS.

Well, from what I experienced, the reps at Best Buy new very little about the deal and had a hard time processing the order. They also told me since this was a new promo for them, there would be some confusion at first. Later, I called Best Buy at the number provided on the promo flyer and they didn't seem to know anything about it and suggested I call Time Warner. Also, according to this post by Bella Spia, the email is actually coming from Time Warner. That rep at Best Buy was sure right about the confusion!
I wonder if iphone24 received the code from Best Buy or Time Warner?

I got the redemption code today by usps mail. But the website asks for uploading copy of previous provider's bill for TV, internet, and phone services. I thought I don't need to provide that if I signed up through Best Buy as new service. What should I do?

It seems from the reward site that if you purchased this promo from Best Buy you have to print out a form and mail it in. Try this link, http://www.twcholidaypromo.com/RewardForm
Click yes and it will take you to the form to print out and mail in. Not sure if this is for the XBox or giftcard though.
It actually says "It’s time to register for your reward card." at the top of form but I do not see anywhere to register for the XBox.
I would try calling Time Warner at 1-888-977-8985 or Best Buy at 1-888-237-8289. Let us know how it goes.

If you go to Terms and Conditions, you will see that this form is for the Best Buy Gift Card. However, it also asks to upload a bill from a previous provider on the Terms and Conditions.

But if you click "No", that you did not purchase the service from Best Buy the form specifically asks to upload a bill from a previous provider. If you click "Yes", you purchased the service from Best Buy the form does not asks for the previous provider's bill. Also, the offer #s are different.

So if we got the service through Best Buy we do not need to show the previous provider's bill. Once I get my code I will call TWC to make sure.

The $150 BB GC is known to TWC reps if you call over the phone. However, the free Xbox 360 offer, I believe they are not aware of that, only TWC BB have that offer.
