Antec Fusion HTPC Case (Silver) $90+shipping
Antec has the silver version of their Veris Fusion HTPC case for $90 + shipping on their site.
I dont think this version includes a PSU, These cases normally take a standard ATX PSU.
This will fit microATX or smaller motherboards
Until 11/30 you can add a Spot cool Fan [antec.com] with your order.

It wont show you shipping charges until you crate a login and use your address. Shipping to CO came out to $11 via fedex ground.
