Sooo, my rebate was declined because of

- Copy of UPC is missing

Even though on the scanned forms of the documents they received there is the copy right there in the corner

I have the same issue with the $20 rebate but when I contacted them they said that i need to resubmit to the same address that was in the rebate form. It's weird that they told you to resubmit to a different address. will they accept a copy of the UPC code since the original was lost?

There are two rebates. One requires the original and the other one requires a copy. I am sure he is talking about the "copy" one, since he mentioned above they wanted a "copy"

Actually, it was for the one with the original UPC. The rebate that requested only a copy of the UPC went through fine. I actually asked them twice just to make sure that a copy would be okay and was told to send the resubmission to the address above.

Did you call them up? I had an issue to with the copy one (thank god it was that one and not the one with original UPC required). Lady was mean in the start (i explained to her I did put the copy of the UPC and double checked and she said no we do it right yada yada) and than I told her I mailed 2 (gave her my tracking number for the other one) and she did a copy of the other one and submitted them and approved them. I am going to do 2 more rebates (from another deal) this time i am going to tape them and even use a glue stick. Also all it has its they been submitted its going to be a pain to wait for those rebates *sighs*.

Has anyone's rebates been approved yet? Mine still says "Documentation Received" after 16 days.

4MyRebate says it will be honored [4myrebate.com]if you include the packing slip to show it was delayed. I would wait until Monday before calling them to make sure it's unambiguously outside the 30 day window if you decide to go through with it. You may also want to take a look at your order status on the site since shipping emails don't go out until the next morning.

Are you sure you sent the invoice rather than the order status page, confirmation email, or packing slip? Does it show up in the scans shown on the tracking page?

In any event you can email them at customerservice@help.4myrebate.com or call them at 1-800-286-9146. If you did include the invoice and they have a blind employee working for them, they'll take care of it right away. Otherwise they'll ask you to resubmit the invoice, probably by mail, but ask about fax if you can send one easily.

Update: Got my $19.99 Amex card. Sent out a resubmission for my $20.00 rebate on the 29th, so hopefully it won't get "lost" in the mail like my last one.
