Depends...Usually corp's tax-id and corporate resolution showing the officer's. Some states and banks are more picky than others and might want to see a copy of the cert of incorporation or some other form of id.

My local branch needed documentation. I went to the county clerk and registered my eBay name as a business. Cost about $50. Took that info and opened the account later that day. You can contact your local branch and tell them you have a sole proprietorship and see if they will accept your social as your tax ID #. I was later approached by Chase (they probably buy list of new businesses) and ended up netting around $2,000 in bonuses for a variety of other new account bonuses.

Other then registering the business in my state and getting a tax id #, is there anything else I'd need to provide as documentation for bonuses like these?

That should be enough. I have opened business accounts with the same info with, ING, Capital One, and Chase. Never had an issue with any of them,

Does the account type have to be "Business Checking" or can it be "Completely Free Small Business Checking"?

Sorry just saw this question. I don't recall what the account was labeled however I never paid a fee and free checks. I've since closed the account and recently opened another business account (different business name) for another $350 bonus with Capital One. No fee's and then sent me free checks, one of those large spiral binder check books, and a deposit stamp, $0 Cost too. Hope this helps.

It was about 10 weeks for my payout. I signed up for the account right at the end of the promotion though.

No mention of actually leaving that 3K in the account though, IE deposit 3K, take out a few days later. Small Business Rewards Checking has $0 and I do not see any disclaimer regarding which specific Business Account is attached to the $300.
