Additionally, it works for future orders, so I placed a couple of orders for the work week, 4 entrees for ~$26, not bad for a 5 day work-week (compared to a regular $5/day budget I use for my lunches, this is a great deal, because I expect the quality to be a bit higher than Subway/local Mexican tacos)

This is a dying chain, they closed all their restaurants in Oregon. When restaurants have to do these kind of deals you know that they're not doing well at all.

It's obvious that most people don't care given the current health disaster. Hopefully that will change one day.

In my ~5 years of experience working in restaurants, they typically do not. I'd estimate ~15% of all take-out orders get a tip.

That said, depending on the restaurant and the way they operate, it often times is quite plausible to be tipping. For example, at Darden chains (Red Lobster, Olive Garden, etc) the bartenders or hosts typically handle the take out orders, which is a lot of extra work compared to your normal job duties, it was very frustrating to not see tips in these experiences. In places with a heavier load of take-out orders (BJ's Brewery & Restaurant comes to mind), they have their own take-out counter with employees staffed to 100% only do take-out orders. In these situations, I consider tipping to be less necessary.

In small, locally owned restaurants, it's almost always the manager or owner who handles the take out, I'd rarely tip in these circumstances.

To your point I was probably going to order the Chicken Under a Brick for dinner. If I was in the restaurant I would have ordered it thinking it was relatively healthy: chicken, asparagus, and roasted potatoes. Then I checked the nutritional information. 3640mg of sodium. The daily recommended intake is 2300mg. Chain restaurants really should put this stuff in the menu.

I'd be more interested in stuff like trans fat and high fructose corn syrup (which helps to gauge how processed the food is) over calorie and sodium counts. I've no problem eating high calorie and high sodium foods as long as I know they are wholesome and nutritious and don't gorge on them. Plus, the correlation between high salt intake and health problems is very weak. Granted, some salts are better than others. For example, the Celtic brand naturally contains vitamins and minerals since it is wet unprocessed sea salt.
