Yeah, you're supposed to place it on the bottom of the page where the sample barcode is. As grifta67 said, it could very easily have fallen off considering how barely stuck on it is.

Personally, I just received word that my rebate has been approved and they are now processing the rebate card! Smilie

Actually, it is taped on top part with the clear tape. The shinny parts you see around the barcode is from the plastic case of the product. It should look something like this.. Smilie

I taped it to the bottom on top of the sample UPC label and pretty much sealed the whole label to the page with packaging tape. You can never be too careful with these people.
Point is, you have evidence directly contradicting what the rebate people are saying. Give them a call and try to work out an option with them.
I had another rebate go through a similar issue (sent an order confirmation instead of invoice, wtf) and they let me resubmit it. It's a PITA, but things seem to be working out for that one too.
