This deal is the same deal as http://slickdeals.net/f/7308320-verizon-fios-triple-play-bundle-tv-internet-phone-400-visa-card-premium-channels-2-years?v=1 but when ordering, in step 1. SELECT/ section 3. Select an Offer you can choose one of the following options

"2-yr Agreement $49.00 mos 1-24"
"2-yr Agreement $59.99 mos 1-24 + $100 Visa card"
"2-yr Agreement $79.99 mos 1-24 + $400 Visa card + Entertainment Pack"
"No Annual Contract $59.00mos 1-24"

These FIOS deals really aren't the great deals they appear to be. We have five TV's in our house & Verizon charges an extra $38 a month to add the 5 TV's?? Why is there an extra charge per month for the TV's? With Time Warner I pay a basic monthly fee no matter how many TV's we add.

Verizon almost never includes STBs in its promos. Maybe Time Warner does?

Dang, this seems like a great deal to me. I recently renewed with Fios and got a $300 Visa card and a tablet if I were to sign up for a 2 year plan on it which I didn't. Wonder if I can call Fios and see if I can get the Entertainment Package added on.

I saw the offer in VA. It could be regional only.

Just ordered. Their system is pretty jacked up. Appointment date I selected shows confirmed at checkout. I then got an email confirming another date for installation.

Total with fees is under $70. This saved me hundreds over the $400 gift card offer over 2 years. I don't care for the premium channels as I don't watch TV much anyhow. Thanks!!

Pretty sure this was a glitch. I don't see it anymore. My confirmation email doesn't mention the price and the website is unable to show my bill estimate. Grr. Gotta give them call now to straighten things out
