Got one from last time.
It's indeed a Nano SIM card with Micro and Standard SIM adapters. Not a single flush piece that you can break off at different sizes.

I bought some from the previous deal, and I find the adapters to be rather fragile. They bent as I was trying to remove them from the frame.

Ebay has the adapters for $1 but with $1 shipping.

Edit : Nvm I'm wrong. One's going for $0.75 free shipping.

I did get one last time. I can see how some people would have difficulty with the adapter card.
Just be GENTLE, and the pieces will come apart; mine did. Keeping the wee card IN the adapter during insertion was a BIT problematic, but it slide right in on the second try.
Getting it activated, on the other hand, was a flippin' nightmare! I had to do it over another phone.
(Trust me; they lost a lot of money getting this card to work. One dollar doesn't even approach the time spent on the phone.)
But, in the end, I was able to activate an old, unused, Unicel phone onto T-Mobile!
Now, to figure out what I want to do with it; I hope I don't lose the phone number for lack of use.
Basic monthly cost is but $3.
