1. It's a one time top off and tax, it's a free plan with no monthly cost.
2. Any New in Box SPP, Volt and Boost will activate off the box sticker. Enter ESN (Hex or Dec) and ICC ID (for LTE), check activated box and go to dashboard, toggle off data in dashboard, boot phone, update OS over wifi, enjoy your new service.
