Newegg Rebate 69792 - Assassin's Creed IV Black Flag & Unity (and Nyko Modular Power Station?)

I bought a Frontpage deal at newegg with a rebate on July 6th and am now turning in the rebate form - yes, at the last minute!

Disclaimer: My question is at the very bottom under the dashed line.

It was for two products; Assassin's Creed, somethin,' somethin', and a Nyko Modular Power Station & Media Remote for Xbox One. I'm unfamiliar with both products other than I do know what the Xbox One is (I even know a new Xbox is about to or just came out, and it's not very different from the last one, 4k maybe?). I just like to buy products when there are good deals- Christmas, birthdays, I've been thinking about getting an...nevermind, I can't be the only impulse buyer watching the Frontpage.
This is probably going to be one of the dumbest questions I've asked in a while (first time forum poster here, but don't worry, asking stupid questions is not something I do for a hobby or to irritate people online or anything), but this question keeps plaguing me so I'm just going to ask the audience.
From what I remember, to get the rebate on the Assassin's Creed game, one needed to buy the Nyko Modular Power Station along with the game or maybe the other way around, just that you had to buy both (and by reviewing the old post, seems a correct assumption), and now I feel like they would need both UPCs to honor the rebate. The rebate form though, seems to me to require that just the UPC from the game is the only UPC needed.
Well, I don't want to lose the rebate because I misread the form.
I'm sure someone is thinking and I've thought too: "I guess I could just cut them out of both products and send them in, to be safe." My problem with that - I don't even want to cut out the UPC on the Digital Download card, because it's unused and what if I do give it as a gift or whatnot.
Because I can't link to the final rebate form because you have to register and then print out a personalized form, blah, blah, I'm going to try and attach a screenshot so someone might be able to see what I mean about requiring just the one UPC - the game is the only product mentioned. Screenshot is attached.
Is anyone out there that filled this rebate form out and, if so, can you tell me what UPCs you sent in? Or anybody that's familiar with this rebate or Newegg's rebates, or just wants to comment could be a help as well.

The rebate only requires the UPC (875873001184) from Assassin's Creed.

The Nyko item has nothing to do with the rebate. It merely was free with the purchase of Assassin's Creed.
