It was slow and getting slower.
Then as things progressed it wouldn't eject. I knew I was in trouble.
If you have a bit of mechanical skill and some time you can fix it yourself if your problem is the same and I bet it is.
When I picked up the PS4 I could hear something rattling around. A loose part.
Firstly, you will need a Torx #9 screwdriver and this will void your warranty. Refer to iFixit for instructions on how to disassemble the PS4. I found the torx screws to be the toughest part, be patient and ensure the screwdriver head is seated before turning.
Once open you will see the BD drive in all its glory. (note this is a launch day version PS4 so the BD drive might be different in newer models, this fix is only for the launch day version. )
Remove the BD drive. Take the power supply out. Be careful of the connectors. Remove the screws holding the BD drive in and again, being careful remove the ribbon cable and the power plug underneath it.
Find the loose part. A tiny plastic rod. Then look at the tapered rollers in the BD drive. You will need to remove 3 philips screws to take the top off the BD drive. Don't worry, it holds together.
Put the rod back between the rollers. There is a groove on either side and the rod fits right in. (terrible design)....
Reassemble. Be VERY careful with the BD drive ribbon cable.
With the device still open test the feed mecahnism. If it is still slow or doesn't engage, this COULD be the ribbon cable. For me, I had to reseat the ribbon cable many times before the BD drive would act "normal"
I am not doing a complete write up here, just trying to encourage you that it is fixable.

The main point I wanted to make here is that the roller repair is simple. Easy. The more difficult part was the ribbon cable reliability. It needs to be perfectly inserted and even then it may act funny if your ps4 is not rebooting all the way.
Test and retest before reassembly.
IF and I say IF you go through everything and the feed is still weak do not give up. Try reseating the optical drive again and retest. This happened to me and I got it back to normal.
