Thanks so much for posting this! The cashier at Kohl's gave me the wrong rebate form.

I spent hours looking online for this rebate but gave up and called Kohls.com customer service and they processed my rebate over the phone, I don’t have to fill out anything she just took all the information from the order I placed. Very easy and quick. I hope it works though. Just another alternative from filling out the form yourself.

Can you submit 2 rebates say 1 for the blender and 1 for the griddle using the same form ?

Form say receive $10 rebate check at one place and at anothe place,
1 rebate form per household.
