The West Virginia Office of Miners’ Health Safety and Training offers the following publications to the mining industry. Ordering information is shown at the bottom of this page.

You can't order this via their online ordering system becuase it's a free publication and the online ordering page only allows for publications that cost something. Eslewhere it does say you can email the Charleston office for free publications so I provided that email address above. You can also sen them a letter or postcard reqeusting the acitvity book.
