I was wondering that too...I filled out a rebate form for $10 back from Rimadyl once...turned out it was $10 off check for next purchase of Rimadyl, made out to the vet. Lame.

Nothing comes up until you close the proprecia website...just make sure to not close the popup. I actually clicked on the popup screen after closing the website...that's when the survey came up.

I think why people are giving you thumbs down is because I saw on another board that this expired 10/31/2008. I didn't see that myself until after I took the survey and qualified. I didn't give you a thumbs down, I thought I would at least give you an explanation as to why they might be doing it.

99% of all the surveys I have ever done were/are for cash.

I did this in September, got the checks (two $5 checks) in October and November, respectively.

This pop up box drove me nuts...I would not let me click on the Yes button. However, I found if you change your screen size from 100% to 50% you can click on it then.
