Just leave the gift code blank. I just signed up for mine (left the gift code box blank) and received a confirmation that they would be sending it out.

is this just for a certain area of texas? it looks like this is concentrated around the dallas / ft worth area? Will they mail to other parts of texas?

No, it's only for new residents of Texas that have just moved into the areas close to the specific hospitals that are listed in the offers.
