Among those successfully COMPLETING the following 10 minute survey (avg), a limited number of individuals will be selected to participate in an on-going community which will reward you with cash and gifts for your participation.

I finished the survey, and at the end, said there would be about 300 spots to fill. Don't know all the specifics, but, pretty sure it is for Diabetes - not sure which Type. (I have Type 2)
Incentives are rewarded mostly as Amazon codes with other "free stuff" and "prizes" available. Requires one 15 minute log-in per week.
