I emailed Colgate and told them I did NOT mail in the form... Got this response...Dear Mrs. xxx:
Thank you for contacting us regarding one of our recent promotions. We
are pleased to have this opportunity to respond.
Due to such a high response rate for this promotional offer, there has
been a slight delay. We have forwarded your request to the fulfillment
house responsible for this promotion. Please let us know if you have
not heard from them within 4 to 5 weeks.
Once again, thank you for taking the time to contact us. We hope this
information is helpful.

For those who completed the program and missed the update....

I emailed Colgate 5 days ago regarding this problem. And today I received a email from them.

Yes, you do need to mail in your form to get the toothbrush. One form for each kid.

* I put in the new thread so you wont miss it. The credit still goes to OP. (ikr227)

 Awesome! My kiddo does this anyways so how great to get her a nice toothbrush. Shes really into this sorta stuff. Rep of course! Also, if you "have" more than one kid.... you can just give them all stars at the same time.... and it doesnt take any longer for say... 5 kids to earn a toothbrush than it would for just one kid...fyi.

I had the same problem last night. I contacted Colgate through email, will post a response once received. That form is incomplete and doesn't even have the number of toothbrushes you are eligible for, so I have no idea how they plan to match up the form to what was done online. The reason is because in step three of the certificate, it states to "CHOOSE ONE" toothbrush. I did this for two, so I don't know if I print out the certificate twice.

From the site:

Way to go! Your family has successfully completed the Healthy Habits nightly log. Each one of your children who has completed the log is now eligible to receive their choice of one Colgate Kids Manual Toothbrush by mail.

If you'd like to submit this redemption form via mail, click here to download and print a PDF version.

^Why does it automatically send you to this version? I am wondering if the online Toothbrush Redemption Form is going to be considered sufficient to get the free toothbrushes, especially since it has each child listed???
-------------------------------------------------------------------------------------------------------------

I sent an email to Colgate consumer affairs. I will post when I get a response.

Hello,
I just completed the Colgate Healthy Habits log for my son. He has achieved 45 stars, and he is now eligible for a free toothbrush. However, the web page leads me to an Official Mail In Certificate (Redemption form). The address to mail the form is incomplete, as follows:

(5) SEND TO:
Healthy Habits Free Manual Toothbrush Offer #
P.O. Box
Strongsville, OH 44149-0301

What is the complete, correct address? There is no offer number and no PO Box number!

Is filling out the online form sufficient, even though it leads me to this Mail-in Form???

Please contact me ASAP.

I got off the phone with Colgate (after being on hold 45 minutes!!!!), and the rep Brian told me the online form IS sufficient. So, there is nothing to mail in.

I hope this helps everyone, since there is no PO BOX number on the mail in form it takes you to. You do NOT have to send anything in. Just compelte the log per child, to earn 45 stars. When you have 45 stars, you can get the free toothbrush via online form.

Finally we got this cleared up!

I called Colgate today and was told the following: Offer # 49190. The PO BOX is also 49190. You will have to submit according to the rep one form for each child since you have to circle which toothbrush that child wants. Then on the address line, you will enter that child's name. The rep also states that you can send one form but indicate how many children and which toothbrush each child wants on a separate letter with the form...in case you don't trust the online thing...doesn't hurt to mail it in anyway...who knows, you may get two shipments :).

Do you ever feel like you are missing something? I'd like to think I am fairly intelligent, but I must be missing the boat on this one. I am on week 3 and have been "logging" and "updating" the stars each day with my children. Yet I don't feel like I am getting any closer to the end. How many stars per child does it take to get to the end? (The toothbrush) How many weeks is the program? (I am on week 3 but there seem to be at least 8 weeks) This was posted at the end of July yet some of you are done and I feel like I am not nearly finished. Any info, suggestions, etc. would be appreciated. Thanks.

one star = number of kids. So, if you have ten kids listed on the program, you will only get 1 star once you click on 10 stars on the log.

When I did the form and hit 'submit', I assumed that they were now accepting online applications. (even tho' I didnt get a confirmation, but got that next page.) The original page does say to click here for a pdf version to print and submit. Thats what made me think you could submit online. That, and the questions they asked and you had to fill in which brushes you chose, etc. Sure hope they do accept the form online.
Thanks for calling and looking into this.

How do my children earn the kids character toothbrush when the log is complete?
Once your family completes the online log, we’ll send you an email notification that you have now qualified for your free toothbrush. Just follow the link provided in that email and complete the redemption form either online or print it and send in the mail. Your kids character toothbrush should arrive within eight to 12 weeks of your submission. Your family will receive one toothbrush per child who successfully completes the log.

When you have 45 stars, you can get the free toothbrush in 10-12 weeks via online form. Finally we got this cleared up!

Well it's not "free" when you have to pay 41 cents for a stamp. (Yes, I'm that cheap). Stupid Colgate. Thanks OP, though.

Also, my form says "P.O. Box" but has no # and says "Offer #" and has no number? Am I missing something?

Oops, just saw AnthonysMama's post

ok anyone else who did it online and did NOT print the form out when it went to that page
can you go into your account now and find the form for printing??
i can not find it anyplace anymore and when i click on Get Your Free Toothbrush
all i see is this now

Way to go! Your family has successfully completed the Healthy Habits nightly log. Your new toothbrush should arrive via mail within 8-12 weeks. In the meantime, check out the other fun games and activities on our site.

so i would think if they want me to send it in even though did it online they would still make the form accessable online?

I was just about to post and ask. I did this back when they offered the battery powered tooth brushes and received those, so i know these will come. If i remember right though the battery ones took only a couple weeks to come, they were alot quicker then they said thats why i wondered on these.

I filled in the online info when it was still messed up, then hit submit. I felt uneasy so I printed the page and added my info then mailed using the address they gave when I called.

So I don't know if these are from online, which they could be as they aren't what I asked for. Didn't the first submit form online not have where you could pick the toothbrushes? I think when I printed the info I filled each one in by hand. I can't remember though.

But like I said what we got wasn't what I ordered...not diego but dora, sb and puss in boots....who knows maybe they'll send more later from the mail ins...;)

I just received all 7 toothbrushes yesterday as I signed up for my kid and his cousins. As mentioned earlier in this post, they each came in separate manila folders via USPS. I submitted the form via mail almost 2 months ago and also did it online just to make sure, but I'm not sure which one worked in the end. But true to their word, the toothbrushes came in 8 weeks. My son is very happy with his Superman toothbrush. I'm sure all who have submitted will get theirs soon. Happy brusing to the little ones.

How can you sign up for more than 1? I don't know how to doit the second time. I also just notice that the Nightly Log is for wehole family, not for each particular child. Am I corrrect? If so, how can I claim free gtoothbrush for more than 1 child? Thx.

when you signed up you should have put all your kid's names in. When you fill out your log there is a place for each kid. Your "points" are an average of all the kids.

I don't think you have to anymore. I believe last year ppl had to mail them in, this year the site was not finished when people completed thier charts and alot of the online options weren't available so people mailed them just as a precaution. :)

Someone correct me if I am wrong though.

"Way to go! Your family has successfully completed the Healthy Habits nightly log. Your new toothbrush should arrive via mail within 8-12 weeks. In the meantime, check out the other fun games and activities on our site."

So how do I get back to the form which the kid and the toothbrush selection. We have to send them in seperately too? One form per kid? that seems like a waste of stamps.

I did it thru email with a nice person and they said they will be sent the week of the 26th and to expect them within 2 weeks after that. I am curious though, when we were able to submit the form online, was there even a link or anything for the mail in form, because I would have done that before the online form if i would have known. I did the mail in form for the last colgate healthy habits with the battery operated ones, and got those. Wish i would have saw the mail in form.......would have been less agrivating.

:shake: :shake: Hmmm....does this mean we have to send them the info again?? I know it's only toothbrushes but the kids that filled the "stars" out everynight were expecting to have the promise that was made honored...It was so long ago that it's been forgotten about around here of course !! I guess it's the principal of it...I already went out and bought the toothbrushes anyway...(they wouldn't have any teeth left if I waited for Colgate)

Oh well...stuff happens !! Thanks for the update:heart:

and that is what i just emailed back to them!

i can not beleive it took them 3 months to tell me that??!! GRRR

also one thing i asked them is how did the company who is supposed to send out
the toothbrused to me not have my information that i gave Colgate when i know Colgate has my personal information because they sent me some coupons last month
so my information is in there someplace that much they have already proved llol
just annoies me because when i asked about this issue in Sept they told me no worries
that the online form was good enough yea right obviously it was not good enough
they had some sort of glitch in their set up

i am soooo glad i never told the kids about the brushes
they too love to get "freebies" and now guess they won't because i too
not holding my breath on this one

good thing we got a few extra from the dentist last month lol

Your response made me go back and check the last I got from Colgate (Dec 19). Obviously, it has been more then 5 weeks since their response. What I got from them was pretty similar to yours, only they didnt' try to claim they had lost my information. Also, consider yourself lucky that you got coupons from them that is more then I have ever gotten.

I completed in mid-August. We never received anything, so I emailed them in the beginning of December. I FINALLY got a response back in early January that told me to send our information (address, name, etc). Then I got another email back the next day that said to wait 4-5 weeks for the fulfillment house to contact me. I never heard anything back. I emailed them again (at the 6 weeks mark - just to give them time). I got a response back on Friday - telling me to wait ANOTHER 4-5 weeks. I never received the coupons you mentioned or anything else. Hopefully this time someone will do something. I still hear everytime the mail truck comes by "maybe my toothbrush will be here today!" It's sad that they disappointed so many kids. I mean, we were told about 3 months - here we are sitting here 6 months later STILL trying to find out where the stupid toothbrush is.

Ditto on this. We completed ours back in October I think..it's been so long ago, I've forgotten. I've emailed them 2x and both responses were the same. "fulfillment is working on this, please allow 4-5 weeks." If they couldn't do what they promised, they should NOT have offered free toothbrushes!

I thought the promotion expired in October 2007? which is when i submitted the online form. I know for sure that it did not state you needed to print it out! Anyways i have yet to receieve them yet either, did get the coupons, but just for money off coupons, not any actual free toothbrush coupons. My nephews are still wondering where there toothbrushes are, I dunno I kinda just give up wih this now.

But, on a positive note, I signed up for their healthy habits for schools and UPS just dropped off a box full of great things for one of my kids classes (not sure why I only asked for one.. I have twins in separate classes, you know one is going to be mad)

Dear Ms. XXX:
Thank you for contacting us over the Internet about Healthy Habits Online Program. Your interest in our products and company is greatly appreciated.
Your response involved material we wanted to send you, therefore the response has been sent via regular (postal) mail and should reach you within three weeks.
If you do not receive this response through regular mail, or have any questions about this, please contact us again by email, or by calling us at 1-800-468-6502, and provide the number appearing after my name below.
Once again, thank you for taking the time to contact us. We hope this information is helpful.
Sincerely,

What are they saying? that they only wanted to respond to me via mail? After months of emails, now they want to communicate via USPS and it could take 3 weeks to reach me?
Obviously what they want to respond with isn't too important.
I'd give up on these guys, but I was impressed with the kindergarten kit they just sent out (both in the speed of delivery and the quality of the materials, my DD class loved it) so I'm not ready to write them off just yet.
