Actually, it's not possible to do the reference - it's all blurred out in one part, and it looks like it's specific to each person. It does say my seq number in the reference which is 407 and then on the form it says also 407 of 485. So I'm thinking they sent out 485 of these.

If it helps, there's a phone number for Fulfillment Systems, Inc on the slip - it's 320-255-0800 - maybe if you call them, they can tell you if they shipped one to you. Sorry that I couldn't help more....
