I got the consent form but it asks for ID # (which I don't recall ever getting but I may be wrong) and Witness' signature (perhaps a MC staff member will sign it when they get the form)?

I got an email from them this morning with a legal consent form attached to fill out and mail in to participate in this study. But I don't know if I want to go through the trouble of printing it off and mailing it in since previous tests didn't require having to mail them a legal consent form. I got this email today because I emailed them yesterday about doing the study. So if you emailed them yesterday check your email to see if you got this legal consent form.

I have no problem with signing the form, but where is our panelist ID# that they ask for? It was not in any emails that I could find.

I emailed them about it, but I only got a form email that was exactly like my first one.

I am confused!

anyone know anything about this??

I had received the e-mail and followed this offer on another site, but the answer is that even tho I sent in the release, I still haven't heard anything back nor have I received the product.

Same here jilly. I never got the sample. I tried replying to their email and it bounced back.

I'm with you girls. Never got anything but today got the email asking me to rate it. I went to the survey, first question was did you get the product, I answered no and then survey ended.

I received the same email from them- but no product to test. I am going to email them and if I hear back I will let you know what they say. There has to be a mix up, how could so many people receive the email but not the product? :)

I didn't get the product to test. Got the follow-up survey today, too. Is that the one where we had to print out a consent form and mail it back to them?
Update:
I took the survey, answered "no" to getting the products. I was re-directed to a sweepstakes form. My info was already entered for me. It said prize value $15. Maybe those of us who didn't get the TP will get something.

Yeah I got the same email. So I sent a question in with name and addy. Then two minutes later they send me a email with a consent form attached to send back "while supplies last" to receive a product sample of Veet...?? Confusing. The email was a reply from the skincare question I sent....I wouldn't mind the sample but it is strange how I got it...

hmm, I still haven't gotten the product to try. When I tried to take the survey, the first question asked what I had gotten, I answered I had not recieved anything, then I was taken to an entry form and when I submitted got an error message stating no longer taking entries.

This is one screwed up PT.

I got the same email. I emailed them back when they sent out the first email with the survey for the product test and told them I never received the product. I never heard back from them. I looked around on their site trying to find a way to contact them but had no luck. Anyone have any ideas?:nod:

i got the invite too.
however when I said that I hadn't read the latest issue, the survey ended, so it was pretty short for me. It ended with a contest entry. Is that how it ended it you didn't get it cut short?

Ok... these people are weird... I got the email a while ago about the waxing PT, mail the consent form but never heard anything back... then today, without any warning, my mail man brings me the waxing PT!! So, it might be coming for others, too! (and it looks neat!)
