I got to the point where it asked for my email. So it is not dead

I just filled it out, and its not dead. It was a pop up and had a survey about current BC and stuff. Then asked for email, and said they'd email if they need you.

It worked for me as well. It didn't say anything about $5, but it did mention the "cash incentive". I used firefox and it said they would contact me if I was chosen for the focus group.

The directions were kind of vague.

I did the discussion for the Nuvaring Hormonal Birth Control. I just got an email saying that my incentive check will be mailed out, and I should receive it in about 6-8 weeks. Im still emailing her back and forth. She seems a little clueless about whats going on. She told me to check my spam folder. Umm why would I do that, when I last emailed her she told me she was mailing a check over a month ago. All my spam has been deleted since she told me to wait for a check in the mail.
