Need help - MIR form for Nextar GPS in Kohls on BF

I forgot to save the MIR form for the Nextar GPS which was on sale for 99.99 with 20$MIR on BF. If anyone has the MIR form as a pdf or something, could you please let me know?

I purchased two nextart X3-03 gps systems from K-Mart on thanksgiving day. The link for the rebate was the wrong one and no one can provide the right form. I talked to Nextar, Nextar Rebate Center, K-Mart, Sears, and I don't know who else. I am crazed right now. I did find the offer code #486-12. Can anyone help me find the rebate form to download? I am lost...thanks to anyone that can help.
