The web page to submit request is no good, it kept saying invalid ZipCode after submit. I have my friends tried their addresses too, they all got the same error. So, we can only email the request then ?

Crap, that form messed up my Firefox! I started typing in the form, and it came out all jumbled, looked like the cursor was jumping around in the first name field. I had to type this in notepad, because now Firefox is doing everything in reverse order!
