How do you get them to apply the subscriptions?

I must have filled these Rewards Gold Business Week forms out a dozen times over the past year and the subscriptions disappear into the ether and NEVER get applied. My subscription is expiring at the end of this month and this is one magazine I'm going to miss. :(

I don't have problems getting other subscriptions through Rewards Gold, but they seem to not want to give me a renewal on this particular one. (Heck at this point a new subscription would work just fine too).
