Can't have annual fee and the rebate always works (not an initial offer with limited time or purchase amount). Is there such a card?

There are quite a few 4 - 5% gas cashback credit cards mentioned on http://ibankdesign.com/board/Cashback_rewards_and_miles_for_personal_credit_car ds_f50.html if you search using keyword "gas". I did not have time to look through them all, but, among those that I checked, penefed cu card looks like the one that fits your description... if you do not mind paying one-time fee for a year of membership in NMFA to become eligible for cu membership.

As Bob already mentioned the Pentagon Federal Credit Union rewards card has 5% cash back on gas.
Associated Bank also has a rewards card much like the old Citi Dividend card that pays 5% cash back at gas stations, grocery stores, and drug stores. Here are the terms:
5% cash rebate on supermarkets, gas stations, drug stores.
1% on everything else.
Max $300 Rebate Dollars every calendar year.
Request checks when reward balance $50 or more.
No annual fee.

You can apply by calling 1-877-577-9485.

Discover was offering 5% cashback from July–September 2009 on gas, it will come around again next year.

If the type of gas you usually buy is BP they have their own Visa card. 10% on gas initially (think for first 90 days), then 5% always. Offers 2% and 1% on other categories of purchases. I think Sunoco offers a similar card. Try looking in stations of the brand of gas you usually buy for promotional offerings.
