I've tried Mini Putt 3 (http://www.addictinggames.com/sports-games/miniputt3.jsp) from Addicting Games.
And the fairly old Mini Golf (http://www.miniclip.com/games/mini-golf/en/) on Miniclips (but probably originated elsewhere).

There used to be one back when the Shockwave website had a ton of free games on in that was tons of fun.
