I've found that the best gifts to a SO are ones that show you truly know/understand them. Sure, flowers/dinner/jewelry is great, but are so general that it could be appropriate for almost anyone for almost any occasion. What does your gf like to do? Is there anything you know of that you don't like to do that she does, and she wishes you would do with her? Also, what kind of budget do you have for this?
One good standby idea is to have a star named for her (http://starregistry.com/). I've done this for gf's in the past and it has gone over well.

Wii. Makes a nice gift.
You can get a nice hookup on Craigslist if she don't mind it being used (Some are new though on there)

i'm a girl... my hubby usually forgoes the gift and does something really fun for my bday. past few years... race formula 1 car at day long race school (very very very awesome!), went skydiving, deep sea fishing.
My advice, is unless she is some premadonna and values the material things that much, take her out and do something really fun!!

What I'm giving my daughter for her 20th; Garmin GPS (SD deal), neoprene laptop sleeve, what happens in vegas DVD; manicure/pedicure with mom & sis, small box of Godiva chocolate, 2 camis from Express (used a SD coupon), Body shop bath gift set.
What I want: a metal detector, a new iPod, a cell phone upgrade, a weekend alone with my hubby.

Wow, your daughter sounds lucky.
But like others have said, what does you gf like to do. How long have you been with her? How old is she? Those can change things.
Oooh a hot air balloon ride would be awesome!

Ohh man the possibilities! Well depends what she is into! But what girl would not love a SPA DAY!!! Pampering! Pedi/Mani!! Get her hair did!! A nice weekend getaway is nice too! I love to gamble so my honey is sending me to Vegas for my bday with my friends!! Last year we drove to Louisiana for my bday weekend as well. The kids enjoyed the pool at the hotel and I had a spa day and then some gambling. Loved every second of it! Yes roses candies dinner pfft that is the norm. Something special and memorable are the things we want!
