So, wherever you find the UPC's... inside the bag, outside of the bag, stuck to the cardboard inside the bag... wherever... just make sure they match the ones on the list I posted. That is all that matters.
All of mine happened to be stuck to the outside of the bag. They were all labels with the UPC info printed on them. But, yours might be inside the bag, or stuck to the cardboard insert. As long as the numbers match the numbers on the list, then you will be fine.
I do understand what you are saying though... if you look at all of the UPC's on the cardboard inside the bag. All three of the 17.3" sleeves have the exact same UPC 023572476721 and two 16" sleeves both have UPC 023572476738.
You should be able to submit one of each of those: 023572476721 and 023572476738, then submit the other three on the labels you have.
If you were to submit only the UPC's from all of the cardboard though, your rebate will get declined, since the rebate offer requires different UPC numbers, and the ones are the cardboard are identical depending on the 17.3" and 16" sleeves.
Attached below are what my UPC labels look like. I also included the UPCs that are on the cardboard of the 17.3" sleeves and the 16" sleeves.

I am so confusing, there is no UPC sticker outside of the. I have 3 UPC sticker inside the bad on top of the cardboard that match the UPC rebate and there are 2 others UPC from the cardboard and it also match the UPC rebate form. So it safe to submit those?

These are the UPC's you want to send in for the rebate for the OGIO slipcovers.
For me, each of the UPC's were stickers (labels) and were stuck on the outside of each bag.
There is another UPC on the cardboard inserts inside of each bag, but those numbers (023572476721 and 023572476738) aren't enough to meet the rebate requirements.
I bought all five of the sleeves. On the cardboard: and all three of the 17.3" sleeves have the exact same UPC 023572476721 and both of the 16" sleeves have the same UPC 023572476738. The problem is that you need FIVE different UPC numbers... and that's where the sticker/labels come in....
These are the UPC's that are listed on the original rebate form information sheet:

You see, this processing center was formerly a shredding service. Unfortunately the first* week of mail arrived before they switched out the equipment...
*The first week's mail arrived on time and was processed promptly. The problems actually began in the next week but I couldn't fit that into this story.
