It says DECLINED, unfortunately. Didn't see that on the status at first. Very strange how this would happen.

Might be worth a call since the packing slip is virtually identical to the invoice -- has the order number, order date, item description, pricing, etc. Tel number is 800-286-9146 (9am-6pm). I've never used the fax number but others have posted that it's 954-573-6751. Someone posted that they received the following correspondence from the rebate company:

You may fax a copy of the Invoice for a resubmission to 954-573-6751. Please be sure to include your rebate tracking number.
