Does anyone have a scan of the upc - Avantquest and Frys are giving me the runaround on these rebates $40 and $60

I have the form but not the boxtop or upc

I do not have a scanner, can you recieve a fax?? if so , give me an e-mail that is not g-mail to contact U .
