I got it, too! It's for a depilatory cream.

I got the consent form but it asks for ID # (which I don't recall ever getting but I may be wrong) and Witness' signature (perhaps a MC staff member will sign it when they get the form)?

FYI: This is for the Marie Claire Velvet Rope Club and apparently they are no longer taking applicants -- at least for the links I found. Congrats to those that got in.
