I just filled it out, and its not dead. It was a pop up and had a survey about current BC and stuff. Then asked for email, and said they'd email if they need you.

try copying and pasting into a different browser like firefox, worked for me after getting the full message

Does not mention anything about the $5 survey anymore. Only a chance to pre-qualify for a focus group.
