After much searching and looking around, finally found the CS number to check the status of the rebate. I had to provide First and Last name and address and the representiative was able to pull up the record. He said that the rebate is currently processing and shall be mailed out soon..... It would have been better if they would have provided the information for tracking the rebate on the FORM.... Oh Well....
BTW: I used the webprint out form and sent the rebate WITHOUT the UPC....

In summary - Users of cheap gasoline may benefit from this product, while users of more reputable sources of fuel [toptiergas.com]might be wasting their time with a cleaning product.
The premise sure sounds reasonable - an engine accumulates buildup over time that degrades performance - remove the buildup, and things should work better.

I used sea foam in the 900 SE turbo and it cleaned it out a bit. Get a bit of throttle body cleaner and a shop towel and you can clean the butterfly valve of the throttle very easily. With sea foam you can take the vaccum lead that is plugged in directly ontop of the manifold off. Start there car and youll see it has a low pressure suction on it. Slowly siphon the can of sea foam into the vacuum line and you car will weez and whine but shouldn't stall. Let it idle for some time then go on a cruze. Youll notice the car will pour out white smoke from the tailpipe, after about 10 mins of driving the seafoam will be completely out of the system and you will have clean vacuum lines and clean engine. Better throttle response, Idle will be smoother, and fuel economy should improve marginally. Pouring things into the gas tank rarely does anything.

That's not really true. Those additives have no effect on the buildup of carbon residue in the intake system because the fuel is injected just above the intake valve, or directly into the cylinder in the case of a DI engine. Buildup of carbon residue in the intake system occurs upstream of the valve and is significant. Buildup on the valve itself occurs over time despite the detergents added to gasoline and the television commercials that tout them.
