As a 5'11" 180lb man with a very muscular build (broad shoulders/chest, 31in waist) I can't find anything off the rack that even comes close - "slim fit" is a joke... Any recommendations or is a tailor the only way? Question applies to dress shirts as well.

Your body shape is (probably) highly atypical. You probably need to go custom. Off the rack suits assume about a 6-7 inch difference between chest and waist (as well as 6 inches from over shoulders to chest), with maybe 2-3 inches of wiggle room for tailoring at the waist. Any time the difference between measurements is more than the wiggle room, you need to start thinking custom.
