For anyone considering the grill pan, I'd like you to also consider that there are essentially no benefits to a grill pan besides the ability to make fake grill marks. With that, you also get a worse sear than a regular (flat-bottomed) pan, a much more difficult pan to clean, and what I guarantee will be the least favorite pan that you own. Enjoy.

Slightly blemished

Not sure how bad the defects. If concerned just spend a few dollars more at Walmart or Amazon

The defects I've seen are the small area on the end of the handle where drips form during the pre-seasoning process, just a spot that's not seasoned (thus far)

if there is a defect, you can send Lodge a picture of the defect and the pan bottom imprint

they will mail you a brand new pan. then you'll have 2 for the price of 1

I have no experience with Lodge warranty. If you must provide receipt as proof of purchase, and the receipt says something like factory defect or blemished, you may be out of luck.

Just looked as well, the order invoices do not specify item quality.

Not if they're being sold AS blemished though, which is what stores like Marshalls and TJ Maxx always do with Lodge products here. It's not just store branding either; Lodge has their own labels made just for "imperfect" stock.
