I have ordered online on 11/12 and received my phone on 11/14. Submitted $200 loyalty rebate on Nov 17. Just called the customer service and she read the terms. Phone order should be placed between 10/16-11/14 and the rebate should be submitted online with in 30 days of receiving the phone or Dec 19, 2020 which ever is early.
I get an error and a message to contact CS.
EDIT : Never mind, looks like the bank account can only be used once ever (had used it for a previous rebate card).
Switched to a different bank account and it went through.
