..... some stores don't have/or know about Heavy All, so you'll have to explain this to them.
If you see they charged you extra for Heavy (anything), politely ask for the manager, ask them if they give you a discount when you tell them No Onions.
Instead of a refund of your extra cost, add some Cents to get a Value fries.

Reminds me of a coworker (a 20 something) who honestly complained to Arby's about his sandwich not looking as good as it did on their billboard. He was not like you, not trying to game the system. He was just naive.
I just shook my head.
For those of us in the real world who know that's just is how it is, we deal with it. I felt like telling him that if he was going to complain every time, he should expect some free special sauce. He would not have understood or believed me.
Their Whopper pictured does not necessarily come with "heavy all". They do use food stylists to make them look perfect. If they advertised them showing them with 4 slices of tomato, they would have to include that for the price, if pressed. Do they painstakingly create a great looking burger? Hell yes! Every visible aspect is made to appeal to us.
Cherry-picking the best-looking toppings and food styling the hell out of the burger is how they get you to really want one. No one would be enthused by the look of the typical standard burger we receive at a fast-food joint. This isn't news, and I doubt it will or should necessarily be mandated to change.
I know when I get a particularly poor burger, and I may complain, but as long as it is as specced, I enjoy the flavor. If I want it my way, I will pay what it costs, if more than standard pricing.
Debating about free extra premium toppings is selfishly creating delays for others in line, and adding stress and work for the staff, and for you.
Will this manager go along easily? Will they spit in my food if they get offended by my Karen attitude?

I agree. They put no love into it. Doesn't taste fresh. Even if it is fresh it's not that satisfying. It's important to think about what makes a good burger, and their bread and toppings are lacking, maybe even their meat and cheese game.
And why can't you add stacker sauce to a Bacon King? I mean on the app. I just don't order if I can't have it my way, I thought that's what Burger King was about.
