I looked the book up on Amazon on my computer and looked at the URL to see the item ID (looks like "B00514HX3F"). Then I said "Alexa, order B00514HX3F". I doubted whether it would work, but it did! Alexa read off the correct title of the book, indicated that the $5 promo had been applied, and confirmed that I wanted to order it.
Hope that helps anyone who might want to use this promo, but is struggling to get Alexa to cooperate.
On any device (including the Alexa app), say this phrase:
She should respond with "(Your book's title) by (your book's author). The Kindle ebook is (price) after a $5.00 Alexa-exclusive ebook credit. Would you like to buy it?"
I've tested a few different books on Echo Dot 3, Echo Show, Echo Show 5, and my Alexa app. This should be foolproof!

There could be a trick to use if you want to combine this deal with gift-giving. I haven't tried it myself, but one could:
Order a super cheap book, .49-.99. Then the remainder should remain as a promotion on the account. For that it appears one can order normally via browser/app. So... I assume that could be used to buy an ebook as a gift?
It's not the full $5, but it seems like it should work for whatever amount remains. But I haven't tested this, and for all I know there could be some weird amazon rule against using any promo at all for gift-giving.
Same happened to me. I did a chat session and they refunded me and withdrew the book.
