If my Ultimate expires in 2022 from the "get 3 years then upgrade it all for a buck" deal, and I add on 1 year of XBL, where do I go from there? Do I then pay again to upgrade that year to ultimate? If so, is it discounted in some way? I'm not really sure how to proceed once I run out...

If you add it now with your Ultimate sub active, it'll extend it by 4 months. If you wait until your Ultimate expires, it'll add the full year of Gold then you can (probably) re-upgrade.

Like the other guy said, let your subscription expire, then redeem this and buy a month of Ultimate. Basically for $65 you'll get a year of GP Ultimate. Way better than $15 a month for 12 months ($180).

I wasn't sure how much the upgrade was. If it's only $15 total and not 15 a month, then I'll probably buy another 3 years and then upgrade! Thank you

This is the newest Ultimate conversion chart, which doesn't yet show the EA nerf that happened recently, 12 months of EA = 2 months of Ultimate now. Your best bet is to let your Ultimate sub lapse, stack Gold then re-upgrade, assuming Microsoft doesn't change the 1:1 conversion ratio in the future. There have been no indications so far that'll happen.

The cheapest way to extend GPU was with 1 year of EA play when it converted to 4 months of GPU. You could get 1 year of GPU for $66. That has since expired since now it converts to 2 months but now the prices are cheaper for 1 year of EA Play. Think I saw $18 the other day so $108 for 1 year of GPU. The best currently is to wait for your membership to expire but no guarantees how long you can do live to GPU conversion.
You can wait until BF and hope for $20 3 month GPU codes again.

The EA conversion has been nerfed to 2 months for a 12 month code. It's not reflected on their conversion charts yet, but it's been confirmed by other SD'ers.

Yup, that chart is for active subscriptions (and as I said, they need to be update it). As of right now, the 1:1 conversion ratio for inactive subs still works, subject to change in the future but I really don't think that'll happen.
