Here is how to get it:

Step 1: Go to www.tellsubway.com
Step 2: Enter the Store Number. You can find it here: http://www.subway.com/storelocator/ NOTE: The free cookie will only be available at the store you enter at this step, so pick carefully.
Step 3: Fill out the survey as you would until you get to where it says "How/where did you pay?" and select "Cash"
Step 4: Enter in a random date/time that has already passed. I am unsure what formats it will take but I used M/DD/YYYY HH:MM:SS and it worked.
Step 5: Enter your email, select if you would like to recieve news/promos for them or not and hit "Submit"

You should recieve an email with the coupon within a few minutes. It will look like this:

In the email it says to write the code on your original reciept and bring it in, but they didn't ask me for mine. I guess YMMV on that, but I don't think they care as long as you have the coupon/survey code since its just one cookie.

Please note that it expires a month after you take the survey. Enjoy!

This is actually a promo for having previously purchased an item. If you look on any Subway receipt you get (at least in my area), it will tell you about this promo and have a place to write the code.

However, you're supposed to be giving them a survey about your last visit. Meaning this is for people that have purchased something. If you just use this to have an endless stream of cookies and never buy anything at a Subway, you are abusing the system.

As I noted previously you actually have to purchase something to get this free cookie, so what you're saying is not possible. Basically, you were supposed to buy something to get this code, but in addition to this you have to buy something to receive the cookie from this code.

I'm a Subway regular so I've always thought you could potentially always get a free cookie for filling out the survey, but that's because I'm actually buying something with it.

Though you're supposed to buy something to get the cookie-survey offer in the first place, I've never heard of needing to buy something to receive it. I may be mistaken though.

got this deal yesterday, after buying a sub in subway ... never felt like this can be a hot deal ... thanks OP. I over looked transaction reference part, just realized that the survey never asked me for one.
