Found this interesting read from our deal friend @fueldude. From what I was told, you'll get a list each quarter to choose from for the 5%.
Can switch between the 2% categories quarterly as well.

New rules as of July 2013: You can't redeem for more than one $25 bonus gift card per calendar year, and you only get up to $2000@5% per quarter. This card is now almost down to being mediocre... barely any better than Chase Freedom or Discover More cards.

Existing customers of US BANK:

Called about a different card I've had over a decade with US Bank.
Casually asked about the Cash+. Rep said that I am already pre-approved for the Cash+; she commented that I would not need a new application or a credit pull if I converted my existing card to the Cash+. Further, said the account number might change but will not show as a new account in the credit report (i.e. the account history/duration stays).
So, if you have a US Bank card you don't use that often...you could just switch.

NOTE: If you sign up for next quarters categories like a day or two before the start of that quarter DO NOT USE THE CARD with the expectation of CB BEFORE the 3 business day wait period as it will not be applied.
US BANK takes 3 business days to activate those categories for you. It is a special circumstance that is easily avoidable with all the time given to register, but in my experience my card arrived at 6/28 so my wait period crossed into the next quarter. As it turned out I got no bonus CB on an early purchase just the 1%.

Was told by USBank CSR they can tell you how a payee is categorized with the payee's name and address/zip, so call before you make the bonus selection or transaction.

So those of us lucky enough to have grandfathered Citi Forward cards paired with the Prestige should be really glad for our 8% on restaurants (when used for AA/US tickets)?

Guys it's only for one quarter that restaurants are going down to 2%. And Fast Food will still be 5%. Just wait a quarter and see what happens.

So far, none of the categories that have been removed (bill pay, airline, hotel) have ever returned.

Where did you see this? I can't find it on my account and I just redeemed $35.00 worth. I was able to redeem the one-time $25 bonus on my first redemption, but I've seen nothing since that time.

I never would have gone for this card if it weren't for it being a product-change from the FlexPerks which had a 0.5% rewards earning rate. I do like that I can use the 5% on my monthly cell phone bill and use another 5% for major purchases planned that quarter - a new bed on the Furniture Store option, for example.

Then you must have a non grandfathered account.

When you go to redeem, it has a little reminder that when you spend 100 you get the bonus.
