I was able to get the form working in IE11 through a successful submission Chrome wouldn't load the page and edge would load the page, but wouldn't go past the date selection on the first page.

The form is very clear in that one needs to submit both the UPC code and sales receipt. See attached screenshot where I marked the step for your convenience.

I chose the online rebate form. Could it be that my sales receipt was a screenshot of my "Thanks for your order xxxxxx" email? Is that not sufficient?
