I just got out of the starting area after a few hours. It will scratch the western RPG itch if you lower your expectations. It is in the KOTOR, Dragon Age or Witcher type of gameplay just much less...

From all the reviews I've seen, this is not open world. There are somewhat large sections of the maps without loading screens, and to move between them there is loading.

Honestly, the reviews seem kinda polarized. I've watched game play though and it seems like my kind of game. Cross between Fallout, Dragon Age, and Assassin's Creed.
