I applied for $24, I taped 3 upc's to a blank sheet of paper. never have I ever included 3 blank sheets with a UPC on each. or labeled them upc 1,2,3

yes just follow the instruction, you will get the rebate. While registering make sure you add 3 quantities, you need to click that ADD button 3 times so that the quantity shows 3.
